A HOUSE RESOLUTION

TO RECOGNIZE AND COMMEND THE ADMINISTRATORS AND STAFF OF THE MARGARET J. WESTON COMMUNITY HEALTH CENTERS, IN AIKEN COUNTY, FOR THEIR OUTSTANDING LEADERSHIP IN THE TREATMENT OF THE MEDICALLY UNDERSERVED, AND TO CONGRATULATE THEM FOR BEING NAMED THE RECIPIENT OF A 2008 JOHNSON AND JOHNSON CRYSTAL AWARD.

Whereas, the members of the South Carolina House of Representatives express sincere appreciation to the Margaret J. Weston Community Health Centers for the vital medical services they offer to citizens of South Carolina; and

Whereas, the Margaret J. Weston Community Health Centers provide quality, accessible healthcare to medically underserved residents of Aiken County; and

Whereas, since 2004, the Margaret J. Weston Community Health Centers have successfully participated in the Bureau of Primary Care Health Disparities Collaborative; and

Whereas, the Chronic Care Health Improvement Project of the Margaret J. Weston Community Health Centers enhances the efforts of the bureau's collaborative to reach the medically undeserved and improve their quality of care through disease management, health education, and proactive patient assistance; and

Whereas, the Chronic Care Health Improvement Project objectives include increasing patient knowledge of self-management for chronic diseases, offering comprehensive chronic care for participants, and providing extensive support services along with intense community outreach; and

Whereas, the members of the South Carolina House of Representatives are pleased to learn that Johnson and Johnson awarded the Margaret J. Weston Community Health Centers the Community Healthcare Award for their work in addressing these medically underserved patients. Now, therefore,

Be it resolved by the House of Representatives:

That the members of the South Carolina House of Representatives, by this resolution, recognize and commend the administrators and staff of the Margaret J. Weston Community Health Centers, in Aiken County, for their outstanding leadership in the treatment of the medically underserved, and congratulate them for being named the recipient of a 2008 Johnson and Johnson Crystal Award.

Be it further resolved that a copy of this resolution be forwarded to the Margaret J. Weston Community Health Centers.
